It is submitted that my Registration Card No: 2016-Z-0000 for session 2014-2016 has been displaced. Duplicate registration card is required to submit in various offices in future.

It is requested that duplicate registration card may kindly be issued in favour of the under signed whereas the fee Rs: 100 under Challan No: 0000000 is deposited in HBL main branch Hafizabad (the copy of the challan in original is attached).
